Work of Heart Nomination Form

The Work of Heart award program was created in partnership with The Catholic Foundation by a donor who sought to achieve a unique charitable purpose.

The award, funded by a grant from The Catholic Foundation, was originally created in 2005 by a generous Dallas family dedicated to Catholic education who wished to remain anonymous. 

The Work of Heart Award Program draws upon the resources, expertise and creativity of The Catholic Foundation to use a designated fund to recognize full-time employees at elementary, middle and high schools in the Catholic Diocese of Dallas.

We are now accepting nominations for the current school year, and appreciate your participation.

During the school year, we are accepting nominations for exemplary full-time Catholic school employees who could receive a $1,000 stipend. Work of Heart awards are presented to “the unsung heroes” who are going the “extra mile” in serving our educational community and its members. The Catholic Foundation encourages participation throughout the Dallas Catholic community.

The details of the award are:

  1. All nominations should be submitted using the fillable PDF nomination form in English or Spanish that you can download from this section, or you may print the form below. Completed nomination forms should be sent to The Catholic Foundation via:
    • email to (preferred method)
    • standard mail and addressed to “Work of Heart” and sent to The Catholic Foundation, 12222 Merit Drive, Suite 850, Dallas, TX 75251
    • faxed to 972-661-0140
  2. Nominations can be submitted on an on-going basis; all nominations will be retained for consideration throughout the current school year.
  3. All required fields of the nomination form must be completed.
  4. Nominations can be submitted by teachers, administrators, staff, chancery employees, students, parents, families, and anyone interested in Catholic education and the well-being of our Dallas area Catholic schools. Candidates may not be nominated by relatives.
  5. Criteria for the award includes:
    • Outstanding service at your school
    • Modeling a Christian/Catholic example
    • Going the extra mile for an individual student, family or the community
    • An individual will not receive more than one Work of Heart Award in a three-year period. Past honorees are eligible again after three years.

Work of Heart Awards that include a stipend of $1,000 are presented to the "unsung heroes” who are going the "extra mile" in serving our Dallas-area Catholic educational community and its members. The Catholic Foundation encourages participation throughout the Dallas-area Catholic community.
